This form will be sent to the Student Counseling Center and will prompt a member of the SCC to contact the student. Please be as detailed as possible in your submission as this will help the Student Counseling Center staff when they make the outreach attempt. The member of the Student Counseling Center will help the student schedule an appointment with a therapist if they are willing to engage in treatment.
