New Requirements for Immunizations
All students must be medically cleared by July 1 to attend classes and not have a hold on their account. All incoming students are required to use Medicat to submit medical/immunization records. Register at https://hamptonu.medicatconnect.com/ using your Infotech credentials: HU username (username is the first section of your HU email address before the @ sign), HUID#, HU email address, First Name and Last Name only. Once registered, take the following steps:
1. Select “Forms” to download your Physical Examination Form and Immunization Certificate to take to your healthcare provider for completion;
2. Select “Immunizations” and enter your immunization history from your completed immunization vaccination record;
3. Upload your Physical Examination Form and Immunization Certificate for Health Center staff to verify your information; and
4. Select “Insurance” to upload a copy of your current Insurance Card.
Upon completion of these steps and after review of your information by the Health Center staff,
your Medical Clearance status will be indicated in your MediCat patient portal within 72 business hours (excluding holidays). A status of Compliant/Verified means that you are Medically Cleared.”

Dear Students:
We are pleased to announce Med + Proctor, an electronic medical clearance platform, as our way to clear all students for enrollment. Med + Proctor is HIPAA and FERPA compliant and your confidential medical information will remain secure.
There is a $10.00 fee to set up your Med + Proctor account. We feel the process will streamline your clearance process and allow you to focus on other important matters as you prepare for a successful tenure at Hampton University. Additionally, the membership provides lifetime access to your immunization data that can be shared with others as you designate.
Once you have set up your @my.hamptonu.edu email account, then you may register for medical clearance through the Med + Proctor site by visiting Medicat and click “register” to start a new account.
We think you will find the website easy to follow. Simply complete the required information and download the medical forms; please ensure the immunization forms are fully completed and signed by your medical provider. Then simply upload your information to the Medicat site. Do not bring your completed forms to the Health Center. You must upload the forms to the Medicat site. You may scan the information into your computer or even upload pictures of the documents straight from your smart phone!
Please carefully note the deadline for your particular Graduate Program. Remember you must set up your @my.hamptonu.edu email address, and then you can register for medical clearance. Please feel free to contact the Health Center at (757)727-5315 if you have any questions.
